4 Southwest is a 56-bed medical pulmonary and telemetry intermediate care unit serving young adult to geriatric patients with a variety of respiratory diagnoses including, but not necessarily limited to: pneumonia, asthma/bronchitis, COPD, pulmonary embolism, stable angina, electrolyte imbalance, atrial fibrillation/flutter, congestive heart failure, and stable patients on ventilators. Full-Time - 1.0/80 Hours Biweekly Evening/Night - 1900-0700 Eligible for Shift Differential General Summary Provides daily clinical support on assigned units ensuring the successful delivery of quality patient care to patients. Collaborates with nursing leadership, and providers on patient placement. Provides direct patient care as an experienced practitioner level, performs duties of charge nurse, preceptor, monitoring, and evaluating nursing personnel during orientation to hospital or unit; assists in planning, evaluating, and revising staff development orientation plans. Duties and Responsibilities + Makes appropriate adjustments/assignments based on clinical care and skills required to maximize timely patient care. + Assists with reviewing developed nursing standards of care implemented based on EBP and nursing research. + Guides the delivery of patient care through prioritizing and organizing work by directing and delegating/or assisting charge nurse with patient care assignments based on acuteness of patient care needs, staff availability and patient safety. Ensures the care provided is consistent with the mission and goals of the system, meeting or exceeding outcomes. + Facilitates and supports safe, quality care at the bedside by providing exemplary direct patient care. Provides direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care. + Collaborates with the care team to adjust nursing care processes as indicated to ensure optimal patient care and Nursing Sensitive Indicators are utilized to drive and improve care. + Collaborates with the leadership team to identify and address operational/clinical/quality problems and other barriers impeding care (e.g., problematic flow issues, equipment and supplies issues, documentation issues, patient satisfaction issues and performance issues). + Anticipates and resolves patient/family concerns in a timely manner. Keeps patients and families informed. + Protects patient confidentiality by promoting professional staff communications. + Serves as a positive resource and role model/mentor to staff. Supports and promotes Professional Governance and encourages staff participation in shared decision-making. + Requisitions supplies and equipment, maintenance and repair service and prepares related forms. Keeps stock and equipment inventories. + Variation of duties may result based on the assigned customer care unit(s). This may include differences in the specific clinical skills required of the RN. Unit based skill lists outline expectations beyond the house wide RN requirements. SYS-NURS-L Nursing Education Requirements Policy outlines current certification(s) required for specific areas of nursing practice and is considered to be part of the job description. + Partners with the Nurse Manager, Nurse Educator and unit-based committees to ensure appropriate orientation, competency and ability of all staff to provide safe, competent care. + Actively participates in the growth and professional development of all staff by encouraging and supporting advancement through the clinical ladder, specialty certifications and supporting continuing education. + Instructs, mentors, and monitors the work of nursing orientees in the aspects of clinical nursing relative to assigned areas; educates nursing orientees concerning nursing policies and procedures. + Collaborates with Clinical Educator and orientee's Nurse Manager in planning and structuring orientation program to meet the individual orientee's needs; plans daily assignments for orientees. + Performs on-going assessment and regular formal evaluations of nursing staff during orientation period; documents all observations and submits recommendations to Nurse Manager (i.e., duration of individual orientation). + Assists Clinical Educator in planning, presenting, evaluating, and revising the staff development activities and orientation clinical plan. + Follows up on S1s, participates and/or leads root cause analysis and process improvement work in unit/department. + In addition to requirements specified in the license and certification section below, current BLS certification for all areas of nursing practice is required.Current certification may be required for the following areas of nursing practice:Emergency Care Center - ACLS, PALSIntensive Care Center & MSCC Lead - ACLS, PALSL&D - ACLS, NRP, - Experienced L&D nurses will obtain within 6 months and new L&D nurses will obtain at 1 year of experienceNICU - NRPPEDS - PALSPCC/PACU/ENDO - ACLS, PALSProcedural - ACLS Common Expectations: + Prepares and maintains written documentation as required by the profession and the department. + Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment.
