UPMC Family Hospice is hiring a Full-Time Home Care Aide for the Lycoming, Union, Northumberland, and surrounding areas + Hours: Monday-Friday 8:00 am-4:30 pm + Weekend Rotation every 3rd-4th weekend and 2 holidays per year + Provide designated care under the direction of the primary service provider in the home to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.Responsibilities: + Direct patient care to the patients in the home setting + Effective communication to patient, family, and team members + Collaboration with interdisciplinary team to ensure a high level of patient safety and customer satisfaction + timely documentation within the patient's home + Must maintain annual competency evaluation scores as determined by regulatory requirements + Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures. + Proven ability to facilitate change. + Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. + Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide. + Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization. + Participates in all required agency meetings. + Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to the appropriate professional staff. + Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards. + Provides safe and direct personal care to patients in the home setting under the direct supervision of an appropriate professional. High School Diploma or GED - Required + One (1) year of experience - Preferred + Certified Nurse Aide- Preferred + Direct patient personal care in the last 5 years + Minimum of 75 hours of Nurse Aide training- Required. If no previous nurse aide training, the applicant may attend the UPMC Home Healthcare Nurse Aide Training Course as available upon hire.Licensure, Certifications, and Clearances: + Automotive Insurance + Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) + Driver's License UPMC is an Equal Opportunity Employer/Disability/Veteran
