Essential to this role are the core values of patient advocacy, holistic assessment, careplanningand the empowerment of patients and families to self-manage.This is a regular PRN position, with caregivers working days between 8:00 a.m. - 4:30 p.m.A caregiver who excels in this role will:Ensure compliance with standards of the accrediting bodies, state mandated revised code and the CMS Conditions of Participation.Promote interprofessional synchronization throughout the acute plete effective handoffs to the next level of care or community to ensure continuity of care.Proactively assess,identifyand work to mitigate the risk posed by barriers to patients’ transitions of care.Work with the patient and/or care partner(s) to develop a comprehensive, individualized transitional care plan.Participate and lead in patient rounding activities,huddlesand committee meetings.Minimum qualifications for the ideal future caregiver include:Graduate from an accredited school of Professional NursingCurrent state licensure as a Registered Nurse (RN)Basic Life Support (BLS) Certification through the American Heart Association (AHA)or American Red CrossThree years of recent RN clinical experiencePreferred qualifications for the ideal future caregiver include:Bachelor’sof Science in Nursing (BSN)Certification by an approved entity within one year of eligibility (CTM, ACM, CCM or AHA)Two years of recent experience inutilizationreview,qualityor care managementKnowledge of acute care and accreditation requirementsExperience in discharge planning and quality improvementKnowledge of EPIC and CarePort softwarePhysical Requirements:Requires normal or corrected hearing and vision to normal range.Ability to perform work in a stationary position for extended periods.Ability to communicate and exchange accurate informationAbility to work with physical records, such as retrieving and filing.Ability to operate a computer and other office equipment.Ability to travel throughout the hospital system.Requires normal or corrected hearing and vision to normal some locations ability to lift up to 10 poundsPersonal Protective Equipment:Follows Standard Precautions using personal protective equipment as required for proceduresPay RangeSalaries [which may be] shown on independent job search websites reflect various market averages and do not represent information obtained directly from The Cleveland Clinic.
