Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Duties include but not limited to: Collaborates with the Boise VA medical facility HBPC Program Director on HBPC Program management responsibilities. Plans and directs the educational and clinical experience of health professions trainees (HPTs) assigned to the HBPC Program. Being readily accessible and promptly responsive to calls from HBPC PACT members with urgent need for consultation, physician orders or as requested by the Veteran. Oversees care planning for all Veterans enrolled in the HBPC Program by either actively participating in HBPC Program team meetings or delegating an HBPC PACT Physician to participate at team meetings. Communicates medical care advances and practice standards to the HBPC PACT. Arranges physician coverage as needed and communicates the plan of coverage to the HBPC PACT. Manages multiple chronic conditions common in older adults, such as diabetes, heart disease, and dementia, ensuring that treatment plans are tailored to individual patient needs and preferences. Works closely with a multidisciplinary team that may include nurse practitioners, social workers, physical therapists, and community health workers. Facilitates discussions about advanced care planning, helping patients and families make informed decisions about future healthcare preferences and end-of-life care. Addresses social determinants of health, such as access to food, transportation and social support, which significantly impact the health and well-being of older adults. Must be available to act as back-up for HBPC PACT primary care providers and make home visits when needed or when the designated HBPC PACT primary care providers are unavailable. Collaborating with other HBPC Medical Directors, VISN leadership and VHA Central Office staff members on program development issues. Work Schedule: Monday-Friday, 8:00 am-4:30pm Additional Requirement: Knowledge of administrative, clinical, and programmatic leadership for clinical care programs to ensure achievement of the clinical goals; Board Certification in Internal Medicine and/or Geriatrics or Family Medicine.
