DescriptionEducates patients about diabetes self-management as well as evidence-based nutrition guidelines in individual or group settings, using approaches that are culturally relevant and age appropriate. Assesses the needs of patient and their family, formulates an educational plan, implements the plan, and evaluates and revises the plan as needed. Functions as a resource to the patient, the community, and other health care professionals both within and outside the Center. Coordinates ongoing patient care and support with appropriate members of the care team. Provides education, training, and support related to diabetes technologies. Plans and conducts continuing education programs in diabetes and general nutrition for allied professionals and the general public. Participates in the assessment, planning, implementation, and evaluation of the diabetes education program at the Center, including continuous quality improvement and other activities related to maintaining recognition with the American Diabetes Association.
