At Cleveland Clinic, you will build a rewarding career with one of the most respected healthcare organizations in the world.Union Hospital is looking to add a Case Management Social Worker to the Care Management department who willcoordinate the care and services for patients within the Emergency Department and Med/Surg floor.Through conducting detailed psychosocial assessments andcoordinatingwithcommunity resources and supportive counseling, you will be able to assess the needs of each patientwho is being discharged. Connecting patients with the proper services and resources can help to set them up for a successful discharge and prevent readmission.When you join our team, you will be part of a supportive caregiver family that will be united in shared values and purpose to fulfill our promise of being the best place to receive care and the best place to work in healthcare.A caregiver in this position works days from 7:00AM - 3:30PMor 7:30AM - 4:00PM, includingrotatingweekends and oneholiday per year.A caregiver who excels in this role will:Manage all aspects of discharge planning for assigned patients to ensurea timelydischarge and provideappropriate connectionwith post-discharge care municate withinterdisciplinaryteam about the discharge planning status of all patients referred.Educate patients,familyand physicianregardingpost-acute options and address freedom of choice and financial concerns.Conduct initial and ongoing psychosocial assessment and interventions and makes treatment recommendations to engage patients and families in adherence to the treatment plan by reducing oreliminatingthe social, psychological, financial,behavioraland regulatory barriers to successful medical outcomes.Serve as a resource person to provide counseling and intervention related to treatment decisions and end-of-life issues.Advocate for patient and family by providing intervention in cases involving child abuse and neglect, domestic violence, elderly abuse, institutionalabuseand sexual assault.Advocate for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system and community.Meet core social work competencies and adhere to regulations.Remain current with regulations whichimpactthe care of patients and regulatory compliance.Promote individual, professional growth and development and meet mandatory and continuing education requirements.Support department-based goals which contribute to the success of the organization.Minimum qualifications for the ideal future caregiver include:Master's Degree in Social WorkorMaster'sof Science in Social Administration (MSSA)Current state licensure as a Social Worker, including LSW, LISWorLISW-SBasic Life Support (BLS)certificationthroughtheAmerican Heart Association (AHA) or American Red CrossPreferred qualifications for the ideal future caregiver include:Three years of experience in healthcare systems to include exposure to and experience in pre-acute careHospital Social Work experienceKnowledge of community resourcesCertification as a Case Manager within one year of eligibility from an approved professional organizationPhysical Requirements:Ability to perform work in a stationary position for extended periods.Ability to communicate and exchange accurate informationAbility to work with physical records, such as retrieving and filing. Ability to operate a computer and other office equipment.Ability to travel throughout the hospital system.Requires normal or corrected hearing and vision to normal some locations ability to lift up to 10 poundsPersonal Protective Equipment:Follows Standard Precautions using personal protective equipment as required for procedures.Pay RangeMinimum Annual Salary: $57,510.00Maximum Annual Salary: $87,697.50The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education.
