This role is ideal for someone who is passionate about whole-person care and building trusted, long-term relationships with members.As a Lead Case Manager, you will work remotely while conducting local, in-person visits throughout Santa Clara County. You will assess member needs, coordinate medical and community-based services, and advocate on behalf of individuals facing challenges related to healthcare access, housing, food insecurity, and behavioral health.
