JOB PURPOSE: The Director of Nursing, under the supervision of the Administrator, provides clinical oversight over all patient care services and staff providing care treatment and services to patients of the agency on a daily basis. KEY RESPONSIBILITIES: u2022 Implement employee relations practices necessary to establish positive partner-partner relationships and promote a high level of partner morale. u2022 Provide oversight coordinating referrals to the agency with internal and external referral sources. u2022 Screen patient referrals for appropriateness for admission. u2022 Evaluate the agencyu2019s ability to meet the anticipated clinical needs of patients prior to accepting the admission. u2022 Provide oversight related to making patient and partner assignments. u2022 Assure the development, implementation, and updates of the patientu2019s individualized plan of care. u2022 Provide oversight regarding all aspects of coordination of patient care delivery. u2022 Assure that patient needs are continually assessed. u2022 Honor patients/residentsu2019 rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints. u2022 Participate in center/agency surveys (Licensure / JCAHO) and any subsequently required reports. u2022 Maintain operations daily workflow by timely completion of tasks assigned to this role within the Home Care Home Based (HCHB) system. u2022 Utilize reporting capability for key indicators within HCHB and Strategic Healthcare Partner to achieve desired outcomes for patients, billing and financial success. u2022 Collaborate with other key partners within organization to achieve agency clinical operational and financial success. u2022 Coach, mentor and serve as a preceptor to clinical partners to achieve positive morale and retention objectives. MINIMUM EDUCATION REQUIRED: u2022 Nursing Degree from an accredited college or university or is a graduate of an accredited/approved RN program. MINIMUM EXPERIENCE REQUIRED: u2022 Two (2) years recent experience as an RN. u2022 One (1) year experience in home health required. MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW: u2022 Current and unrestricted Registered Nurse (RN) licensure in state of practice. u2022 Current CPR certification.
