We want your knowledge and expertise Job Summary and Qualifications The advanced clinical pharmacist practices the profession of pharmacy with the utmost code of ethics demonstrating the organizationu2019s mission and values. The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The clinical pharmacist serves as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. In addition, the advanced clinical pharmacist is expected to lead pharmacy services within the hospital by serving or chairing hospital committees, presenting at conferences and contributing to publications, and providing regular, formal education to pharmacy and interdisciplinary teams. The advanced clinical pharmacist is responsible for one or more specialty services within the hospital, such as infectious disease (ID), critical care, and pediatrics, among others. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the practice of the clinical pharmacist. + Leads committees responsible for establishing and implementing medication-related policies and procedures (e.g. Pharmacy & Therapeutics, Medication Safety, Antimicrobial Management Program) + Demonstrates leadership within the pharmacy department through a documented record of improvements in and contributions to pharmacy practice + u200bMaintain ongoing commitment to advancing the profession in the area of pharmacy practice in which they serve + u200bProvides regular, formal education to pharmacy and interdisciplinary teams in their field of expertise. + u200bModels, mentors, and encourages pharmacy students, residents, and colleagues in a manner that fosters highly professional pharmaceutical care and high personal standards of integrity. + u200bPerforms all responsibilities of the clinical pharmacist as outlined below. + u200bVerifies Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness. Intervenes when orders are unclear or potentially unsafe or inappropriate + u200bInterprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Follows facility practices and procedures for documentation in the patientu2019s medical record. Trouble shoots/clarifies Centralized Order Entry (COE) orders. + u200bPrepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques, USP 797 & 800 guidelines. + u200bEvaluates Total Parenteral Nutrition (TPN) for appropriateness based on facility policies and guidelines or ASPEN guidelines or recommendations as appropriate + u200bIssues controlled substances to patient care areas and maintains records as required by law. In addition follows corporate policies and procedures related to the dispensing and monitoring of controlled substances. + u200bEnsures safe, appropriate, evidence-based, and cost-effective medication therapies for patients according to established policies, procedures, and protocols + u200bMonitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, indications and therapeutic duplications + u200bBased on knowledge & expertise, develop, assist, implement, and execute all drug therapy management protocols including, but not limited to; pharmacokinetics, anticoagulation dosing, IV to PO, pain management, antimicrobial stewardship, and pharmacy-directed dosing. + u200bReviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed for drug-bug mismatch, de-escalation and duration of therapy. + u200bReads, extracts, and interprets information in patient charts accurately + u200bDetects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner. Shares good catches and clinical outcomes to pharmacy leadership to elevate pharmacist interventions and services. Reviews medication related events that have occurred to prevent recurrences. + u200bSustains the formulary by instituting drug therapy modifications for formulary compliance, SKU reduction and management of shortages as applicable. + u200bEvaluate and respond to patient-specific drug information questions from clinicians. + u200bProvides drug information to patients and their families/performs applicable medication reconciliation. + u200bEnsures CPW rules/alerts are being reviewed and completed in a timely manner. Utilizes Vigilanz/CPW to document clinical interventions. Communication for consults, documentation for the permanent medical record or for other disciplines will occur in the permanent electronic health record. + u200bDocuments all required double checks per facility practice + Participates in the quality improvement and medication-use review activities of the department. Collects data; conducts quality monitors and inspections; and maintains logs, records, and other documentation as assigned + Participates in rounds and/or direct follow up with providers. This may include interdisciplinary rounds, rounds with individual providers, onsite/in person follow up with providers, and via phone or written communication based on urgency/ability to meet with provider in person. + u200bParticipates in the nursing, physician, or ancillary team onboarding and staff meetings as appropriate to explain clinical pharmacy services and educate on new drugs or updated clinical guidelines. + u200bSupervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel + u200bMaintains current pharmacist licensure, and maintains competence required for current job title/position. Pursues board certification or other training/certification for clinical area of expertise based on job role. + u200bParticipates in local and/or national pharmacy organizations, and pursues research, publication and speaking opportunities. + u200bAttends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of pharmacy + u200bMaintains strict confidentiality of patient, visitor, and employee information. Complies with HIPAA Privacy Rule standards. + u200bIs able to work with automated systems. Is granted Automated Dispensing System (ASDS-Pyxis) /Meditech access and privileges. . Potential certifications may include: SIDP, MAD-ID, CPPS, among others. Required HCA Florida Poinciana Hospital is a purpose-guided healthcare provider. We are a four-story, 150,000-square-foot hospital. We have 70 private beds and a six-bed private ICU. We provide a full range of acute care services including emergency services, inpatient and outpatient diagnostic imaging, and inpatient and outpatient surgery. Other services include cardiac catheterization, laboratory, pharmacy, and a full range of support services. Our Emergency Department (ED) is approximately 14,500 square feet. Our ED consists of 22 private treatment rooms, an EMS entrance, and a helicopter pad.
