Summary: A Nurse Practitioner/Physician Assistant functions as part of the interdisciplinary healthcare team in accordance with privileges approved by the credentialing committee to provide high quality, cost effective care to patients within the APPs scope of practice in collaboration with a supervising physician. The APP reflects the mission, vision, and values of the organization, and complies with all relevant policies, procedures, guidelines and other regulatory and accreditation standards. This role provides the following opportunities: u00b7Work in a team environment to provide high-quality inpatient care to patients in their own homes through Advanced Care at Home (ACH). u00b7Clinical outcomes are equal to or better than inpatient hospitalization with improved patient experience u00b7State-of-the-art equipment at Command Center and patientsu2019 home u00b7Patients have access to hospital services and more including medication management, mobile imaging, home infusion, meal delivery, nutrition consults, phlebotomy, rehab services u00b7Integrated care with primary care providers, specialists & ancillary departments Responsibilities: u00b7Perform the initial virtual history and physical for new patient admissions after patient arrives home u00b7Collaborate with the supervising physician to develop and implement a plan of care, including diagnostic and specialty consultations virtually or as day trips to brick and mortar facilities u00b7Travel to patientu2019s home and perform in-person assessments, as clinically indicated u00b7Provide safe handoffs to the attending physician and Command Center Registered Nurse, as well as patientu2019s primary care physician during hospitalization as needed and at discharge u00b7Perform clinical screenings on eligible patients at eligible brick and mortar facilities u00b7Communicate with the patient and family concerning the ACH model, incorporating patient and family values and goals in the plan of care u00b7Promote a collaborative relationship between the physicians, Command Center Registered Nurse, and supplier providers u00b7Maintain and practice in accord with practitioner guidelines per state and hospital regulations u00b7 Command Center u2013 Parkline - is considered u201cplace of businessu201d for this role located in Chapel Hill, NC.
