FTE: 1.000000 Standard Hours: 40.00 Shift:1 Schedule: first shift (generally 8am-4:30pm) working Monday throughFriday Job Summary: This Clinical Ambulatory Pharmacist will supportboth Endocrine and Primary Care/Population Health. The pharmacist willhave dedicated days each week spilt between the Endocrine and PrimaryCare/Population Health clinics. This pharmacist will be part of aclinical pharmacy team in the FMLH Endocrine clinic that provideschronic disease state management for type 1 and type 2 diabetes;osteoporosis and osteopenia; as well as weight loss. Within PrimaryCare, they will help support population health initiatives. Thispharmacist will serve as a medication resource, will help with theoptimization of patients' medication regimens, and may have otherresponsibilities, as assigned. In this role, the pharmacist will providedirect patient care via in-person office visits, virtual visits, andtelephone encounters. This is a hybrid position and will require thepharmacist to travel between clinics. Using independent decision makingand critical thinking skills, the Pharmacist promotes medication usethat is safe, effective and considers the costs and benefits of therapyto maximize value. The Pharmacist interacts with other members of thehealthcare team and with patients to proactively address medicationtherapy issues. The Pharmacist ensures patient care is maintainedthrough compliance with the department's quality improvement plan, andthe Joint Commission/Center for Medicare/Medicaid standards forpharmaceutical services. EXPERIENCE DESCRIPTION: A minimum of 1 yearhospital or 3 years of other pharmacy experience is preferred. EDUCATIONDESCRIPTION: Bachelor's degree in Pharmacy or Pharm.D. from an ACPEcertified school of pharmacy is required. Successful completion of anASHP accredited PGY1 or specialty residency is preferred. SPECIAL SKILLSDESCRIPTION: Demonstrates effective use of computers and applications(e.g. MS Word, Excel, Outlook) as appropriate to practice andcommunicate effectively. Active membership in the Pharmacy Society ofWisconsin (PSW) preferred. Active membership in the American Society ofHealth System Pharmacists (ASHP) and/or other national pharmacyorganizations as approved by the Pharmacy Administration preferred.LICENSURE DESCRIPTION: Active Wisconsin Pharmacist License is requiredor eligible for and must obtain within 6 months.
