Employment Type: Full time Shift: Description: ESSENTIAL FUNCTIONS: u00b7 In collaboration with multidisciplinary team, evaluates patient referrals to develop individualized plan of care across the continuum of care u00b7 Responsible for personalized education plan for patient and family u00b7 Follows through with test results and plan of care with patient/family and other health care providers/services as directed by provider or established guidelines u00b7 Acts as a liaison among the individual/family, community, provider and physician as well as other health care team members. u00b7 Collaborates in developing, implementing and evaluating teaching/learning strategies for client/family in the hospital community. u00b7 Provides clinical leadership by being a preceptor for visiting professionals, students and new graduates. u00b7 Coordinates appropriate educational material/instructions for patients, families, medical staff and multidisciplinary personnel to accomplish patient care outcomes. u00b7 Participates in planning and presentation of continuing education programs, seminars and meetings. u00b7 Coordinates with the health care team to develop a mechanism to monitor patient outcomes and follows up on variances. u00b7 Ensures collection and input of risk stratification data. Reports results to the health care team and participates in interpretation of data and formulation of action plan. u00b7 Collaborates with patients/families/populations to coordinate community services and appropriate resources which meet the individual needs and goals. u00b7 Serves on selected hospital, group practice and community-based committees. u00b7 Identifies need for and participates in research projects relevant to specific patient population. u00b7 Collaborates with Quality Management, Utilization Review, Information Technology Services, and other departments regarding data availability and reports. u00b7 Is accountable for the implementation and communication of standards for patient care delivery appropriate to the specific patient population that meets professional and regulatory standards. u00b7 Coordinates the health care team in development of clinical tools for optimal outcomes and reports findings. MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: u00b7 Current Iowa registered nurse (RN) license. u00b7 Bacheloru2019s degree in health-related field required or actively pursuing. BSN preferred. u00b7 Demonstrates competency within a specialized area of clinical practice with 4 years experience in that specialty. u00b7 Proof of completion of Mandatory Reporter u2013 Child and Dependent Adult Abuse training within three (3) months of hire. u00b7 Basic Life Support certified within six (6) weeks of hire. Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support for Healthcare Providers. u00b7 Demonstrates success in positions of leadership.
