Community Health Navigator We are seeking a Community Health Navigatorto join our multicultural New American program team. This staff memberwill serve as a client case manager and play a pivotal role in fosteringconnections and collaboration between JFSWM and the refugee andimmigrant community. This position involves developing and implementingstrategies to engage community members, addressing their needs, andpromoting social integration. Position Details: Position Title:Community Health Navigator Department: New American Program Status:Non-Exempt, Full-Time Reports to: Health Promotion Program CoordinatorPay Range: $20-$22/hr Location: Springfield Responsibilities: 1.Health Case Management and Navigational Support -Conduct thoroughintakes, eligibility screenings, and progress assessments to identifyhealth-related needs and concerns for a caseload of clients. -Completenecessary follow-up activities, supporting clients to scheduleappointments and ensure they get the appropriate care needed. -Assistclients in understanding medical terminology, navigating the healthcaresystem, interpreting health-related documents, and advocating for theirhealth needs. -Complete all necessary documentation of work withclients, including case notes, intake paperwork, and service plans. 2.Education and Group Programming -Conduct outreach activities to raiseawareness about preventive health measures and available healthcareresources to JFSWM clients. -Provide education and training sessions onhealth-related topics to community members, such as basic health carenavigation, chronic health condition management, nutrition education,etc. 3. Elder Adult Case Management -Provide guidance to older adults onthe path to citizenship. -Support elders through cultural orientationand adjustment to life in the United States. -Connect clients withcommunity resources that promote health, safety, and independence.-Foster opportunities for social integration and engagement within thecommunity. Qualifications: -Bachelor?s degree in social work, publichealth, human services, or a related field. Experience working withrefugee populations is highly desirable. -Knowledge of refugee healthissues, commitment to cultural sensitivity, and experience workingwithin diverse communities. -Demonstrated ability to workcollaboratively and independently, to plan and organize work, askrelevant questions, and follow tasks through completion. -Knowledge oflocal resources, social services, and agencies in Western Massachusetts,specifically those serving immigrant populations. -Fluency in Englishand another relevant language is preferred (Swahili, Haitian Creole, orDari/Pashto preferred) Previous experience in community engagement,preferably within refugee or newcomer communities or similar contexts.Demonstrated ability to plan and organize community events andinitiatives. A valid driver?s license, with an ability and openness tousing personal and agency vehicles. A driving history check will beperformed for the final candidate.
