Summary The Full-Time 1.0 FTEE Registered Nurse Cancer Care Coordinator position is available within the Ambulatory Care - Outpatient Nursing Division. This position is located at the Clement J. Zablocki VA Medical Center in Milwaukee, Wisconsin. Milwaukee is on the shores of Lake Michigan, the fifth largest lake in the world where Summerfest, the world's largest music festival. Responsibilities The Cancer Care Coordinator Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients. The RN provides patient management, care coordination, and disposition planning for cancer patients. The duties include but are not limited to: Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinate care with multiple providers across all levels and sites of care. Assuring service interface within the Veterans Affairs (VA) and community programs to provide continuity of care to patients. Provides oversight of patient's journey through the diagnostic and treatment continuum, process is scheduled and completed, while tracking the overall treatment plan. They will follow certain patients from the earliest suspicion of cancer through the continuum of cancer care including diagnosis, treatment, survivorship/surveillance, and palliation/end of life. Will interface with outside referral agencies and ensure that their business/clinical practices follow compliance and regulation standards. Anticipates and implements preventive patient care measures. Assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs. Provides guidance and education to patients and caregivers and collaborates with other professionals at all levels. Permanent Change of Station (PCS): Not Authorized Requirements Conditions of Employment U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
