Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space. Communicates through various digital channels to support patient care. Performs initial health screens and intake for new members. Obtains and reviews old medical records to ensure comprehensive patient history. Identifies care gaps and orders necessary screening tests. Confirms appointments and provides instructions to patients. Conducts pre-visit RAF reviews to prepare for patient visits. Welcomes patients, provides tours of the practice, and reviews welcome packets. Ensures all medical records are available for office visits. Performs needed screenings and orders appropriate tests during visits. Conducts intra-visit RAF reviews to assess patient needs. Reviews quality metrics, performs vitals, screenings, and medication reconciliation. Manages the check-out process with patients and conducts post-visit follow-up calls. Other duties as assigned.
