Summary The Ambulatory Care Patient Aligned Care Team Licensed Practical Nurse is an Intermittent LPN position within the Women's Health Clinic at Martinsburg VA. The incumbent functions as a member of the health care team by demonstrating an exceptional degree of practical nurse knowledge and skills necessary for self-direction in carrying out complex practical nurse care assignments within the CBOC's /CPC's for a Veteran population with a variety of health care needs. Responsibilities The incumbent collaborates with a PACT team to provide complex/specialized patient centered care to a panel of approximately 1200 patients taking into consideration patients age, cultural backgrounds, education level and any other special needs. The LPN/LVN is responsible for goals and interventions that promote veterans in attaining maximum functional potential. This is accomplished through but not limited to modalities such as ambulation, bowel and bladder training, dressing, grooming, bathing, oral hygiene, range of motion exercises and fall prevention modalities. Accurately and consistently reports and records significant veteran information received from other team members and can analyze and implement modifications based on these reports. Promotes and demonstrates nursing concepts in the Medical Center. Works in collaboration with the Registered Nurse (RN) and or a Health Care Provider (HCP). Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular, and/or intravenous) and performs treatments according to established policies/procedures. Assists other staff to prioritize the overall veteran care needs and adjusts the plan of care as needed. Responds diligently to the veteran changes and deteriorating condition in emergent situations, notifying appropriate personnel in a timely manner. The incumbent must exercise independent judgement and requires minimal supervisory oversight. Supervisory oversight for the PACT LPN is the clinic Nurse Manager. The LPN must possess the knowledge and skill to promptly recognize potential urgent/emergent patient care situations, seek team assistance as appropriate, and effectively intervene to assure the delivery of safe patient care.
