JOB PURPOSE: Responsible for coordinating and directing Social Services in accordance with federal, state and local regulations, established procedural guidelines and as directed by the Administrator. KEY RESPONSIBILITIES: 1. Applies standards of professional Social Service work practice. 2. Demonstrates knowledge and respect for the rights, dignity and individuality of each resident in all interactions. 3. Demonstrates competency in the protection and promotion of and advocacy for residentsu2019 rights. 4. Responds to resident behavioral and psychiatric issues by completing behavioral and psychosocial assessments, providing treatment recommendations and making referrals to appropriate mental and behavioral health providers. 5. Coordinates discharge plans with residents and families. 6. Assists patients and families with advance directives in accordance with Federal regulations. 7. Resolves and tracks grievances. 8. Completes psychosocial assessments of residents. 9. Coordinates family council meetings. 10. Coordinates ancillary appointments as necessary. KNOWLEDGE, SKILLS, ABILITIES: u2022 Participates in center/agency surveys (Licensure/JCAHO) and any subsequently required reports. u2022 Attends and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification as required. u2022 Attends and participates in mandatory in-services. u2022 Honors patients/residentsu2019 rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints. u2022 Complies with corporate compliance program. u2022 Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary. u2022 Follows established safety regulations, to include fire protection and prevention, smoking regulations, infection control, etc. u2022 Follows established safety procedures when performing tasks and/or working with equipment. u2022 Performs other related duties as necessary and as directed by supervisor. u2022 Customer service oriented. u2022 Should possess good interpersonal skills. u2022 Communicates well with patients/residents/clients and family members providing warm and friendly greeting and an approachable attitude to families, visitors, patients/residents/clients and responds to expressed concerns while displaying a helpful, caring demeanor. Answers questions when appropriate in a professional manner. MINIMUM EDUCATION REQUIRED: For buildings with more than 120 beds: Minimum of bacheloru2019s degree in Social work, or Human Services field such as Sociology, Gerontology, Special Education, Rehabilitation, Counseling or Psychology. MINIMUM EXPERIENCE REQUIRED: Minimum of one year of supervised social work experience in healthcare or long term care setting working directly with patients. Previous supervisory experience.
