What Youu2019ll Do: + Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and facilities. + Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership. + Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers. + Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments. + Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers. + Utilize Bloomu2019s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes. + Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.
