Job Title: Food Access Coordinator Entity: Clinical Practices of the University of Pennsylvania Department: Puentes de Salud Location: Penn Medicine Rittenhouse- 1700 South Street Hours: Mon-Fri Full Time per department needs Summary: Puentes de Salud (Bridges of Health) is a 501(c)3 nonprofit organization that promotes the health and wellness of Latinx immigrants in and around Philadelphia through high-quality health care, innovative educational programs, and community building. The foundation of Puentes de Salud rests on the belief that community wellbeing results from a collaborative, multidisciplinary effort that focuses on addressing the conditions in which we grow, learn, live, work and age. The Food Access Coordinator collaborates with wellness team members to plan, implement, and manage Puentes de Salud's food access programs and services. This position reports to the Health & Wellness Director. Responsibilities: + Collaboratively lead Puentesu2019 food access efforts with clients, food partners, and Puentes staff. + Coordinate direct food access services for patients and clients. This includes conducting surveys, phone calls, and some in-person services. Conduct referrals, follow ups, and ensure that patients and clients are connected with needed resources. + Contribute to primary relationships with Old Pine Community Center and other food access groups. + Serve as consistent point of contact for community members in need of food support, as well as for Puentes staff to refer clients/patients in need of food support. + Support the Food Pharmacy, Puentes' on-site food distribution site, including managing inventory of food and hygiene items, bottom-lining organization and stock rotation, and collaborating with a team of volunteers, Promotoras, and other staff to provide coverage of the Pharmacy during clinic hours. + Contribute to recruitment and training of paid and volunteer drivers to deliver food to clients. + Directly participate in service delivery by serving on an on call/as needed basis as driver to deliver despensas to clients + Collaborate closely with staff team to assess and troubleshoot food access work. + Collaboratively lead efforts to refine and evolve food access programs, focusing on priorities defined in Wellness plan + Collaborate across disciplines and service areas to coordinate with staff including Promotoras, contractors, clinical staff, students, and volunteers. + Develop and shepherd relationships with related external partner organizations. + Conduct data entry and help maintain and analyze qualitative and quantitative service data. + Participate in resource development and grant-writing to sustain Puentesu2019 food access programs. + Assist with the development and implementation of other new and existing community-based wellness initiatives and outreach. Education or Equivalent Experience: + Bachelor's Degre (Required) + 2+ years experience (Required). Skills & Abilities: + Prior experience with immigrant populations, program development and coordination. + Oral and written fluency in Spanish. + Proficiency in Microsoft office. + Strong problem-solving, communication, planning, and organizational skills. + A valid driveru2019s license and access to a personal vehicle with current registration and insurance.
