JOB REQUIREMENTS: Group Health Cooperative of South Central Wisconsin.BETTER TOGETHER This is a full-time hybrid role (40 hours per week).Candidates must be residents of Wisconsin. Under the direction, guidanceand general supervision of the Population Health Manager, the PopulationHealth Specialist plays a crucial role in coordinating and supportingGHC-SCW's population initiatives, community partnerships, communityhealth programs, and social work services. They actively contribute tovarious community health programs, including the Bridges to AccessProgram, Stay Healthy Visit Program, Refugee Assistance Program, PrimaryAccess for Kids Program, and Financial Assistance Program. In additionto program involvement, the Population Health Specialist aids in thedevelopment, monitoring and sustainment of partnerships and charitablecontributions. The position actively supports the organization's healthequity initiatives for both members and patients. This position is alsoa key contact for GHC-SCW clinic staff that assist in identifying anddirecting patients to other community services and resources that may beavailable. Job Specific Minimum Qualifications Bachelor's degree from anaccredited college or university in a relevant major, or equivalentcombination of relevant education and experience is required. A minimumof two (2) to three (3) years of experience in an office or healthcareenvironment including customer service is required. Direct experiencewith community services and/or Medicaid or BadgerCare population ispreferred. Previous experience using the telephone in a service capacityis preferred. Experience with medical terminology is preferred.Knowledge of and proven proficiency in the use of Microsoft Officeapplications including Word, Outlook, Excel, PowerPoint, Visio isrequired. Proficiency in SharePoint is preferred. Possession of currentState of Wisconsin Driver license with a good driving record, access toa vehicle, and the ability to travel is required.
