JOB SUMMARY: We are seeking compassionate and dedicatedprofessionals to join our team as Care Coordinators in our Youth DualCare Coordination program. This program supports children and theirfamilies in Milwaukee County who are facing challenges with severemental health and/or substance use disorders, with co-occurringdevelopmental and/or physical disability needs. Dual Care Coordinationprogramming follows the wraparound model of service delivery to identifyneeded services and develop multi-disciplinary treatment plans thataddress both the strengths and needs of the family. As the CareCoordinator, you will manage a caseload of youth who are dually enrolledin Comprehensive Community Services (CCS) and Children's Long-TermSupport (CLTS) programs. You will coordinate in-home and community-basedservices to support the families' individualized needs. KEYRESPONSIBILITIES OF THE CARE COORDINATOR: Provide case managementservices to a caseload of 12-13 clients. Interview and evaluate clientsand families during the intake and assessment process to analyze clientand family needs and strengths. Create and implement goal-oriented andstrengths-based treatment plans. Facilitate team meetings with theclient, family, and direct service providers to support and monitorprogress of goals and services. Develop a crisis/safety plan for theclient and family. Develop a family support team comprised of formal andinformal supports. Work in collaboration with other providers, schoolpersonnel, mental health professionals, Child and Family Servicesworkers, and court officials. Exercise judgement to intervene as anadvocate for clients in emergency and crisis situations. Safelytransport clients as needed. LOCATION: West Allis, WI. Local travel isrequired to meet with clients within their home and other communityspaces throughout Milwaukee County and surrounding areas, with mileagereimbursement. SCHEDULE: Monday-Friday 8:00am-5:00pm, with occasionalevenings to accommodate families' schedules.
