Jordan Health is now seeking aRN Triagewho, under the guidance of the Director of Nursing, is primarily responsible for working collaboratively to identify and proactively manage the care needs of patients within the Family Medicine practice setting. TheRN Triageprovides assessment, care coordination, advocacy and coaching for patients. Education And Experience Required: u00b7 1 year of nursing experience required. u00b7 Preferred experience in urgent care, emergency department, pediatrics, or OB/GYN. Licenses And Certifications: u00b7 Current NYS RN License without any negative actions, BSN preferred. u00b7 Current provider BLS/CPR through the American Heart Association. u00b7 Infection Control Certification. Special Skills, Knowledge Required: u00b7 Strong computer skills. u00b7 Excellent customer service skills both phone and in person. u00b7 Ability to work well with others in a multi-disciplinary team. u00b7 Ability to organize and multi-task.
