Responsibilities Work Schedule: Monday - Friday, 8.5 - 9 hours between 7:00am and 7:00pm, to include a 30 - 60 min uncompensated lunch break No Weekends or Holidays No Overtime Job Specific Details and Tasks: Duties may include the following, dependent upon department: u2022 Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness. u2022 Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF. u2022 Perform physical exam and health histories. u2022 Provide health promotions, counseling, and education. u2022 Administer medications, wound care, and numerous other personalized interventions. u2022 Direct and supervise care provided by other healthcare professionals. u2022 Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel. u2022 Recognize adverse signs and symptoms and quickly react in emergency situations. u2022 Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization. u2022 Make referral appointments and arrange specialty care as appropriate. u2022 Conduct research in support of improved practice and patient outcomes. u2022 Attend staff meetings to provide case discussion, continuing education, and continuous quality improvement and safety. u2022 Ability to effectively utilize an electronic health record and other software applications, or revert to paper documentation if systems are unavailable, to appropriately document assessments, interventions, and evaluation of patientsu2019 plans of care. uf0b7 Maintain a level of productivity comparable with that of other individuals performing similar services. uf0b7 Participate in peer review and performance improvement activities. uf0b7 Practice aseptic techniques as necessary. Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection. uf0b7 Function with an awareness and application of safety procedures. uf0b7 Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Anticipate potential problems/emergencies and make appropriate interventions. Notify supervisor, leadership, or other designated person regarding problems that the HCW is unable to manage. uf0b7 Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner. uf0b7 Participate in the implementation of the Family Advocacy Program as directed. Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting. uf0b7 Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF. uf0b7 Maintain an awareness of responsibility and accountability for own professional practice. uf0b7 Participate in continuing education to meet own professional growth. uf0b7 Actively participate in the commandu2019s Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist. uf0b7 Participate in the provision of in-service training to clinic/ward/unit staff members. Assist with providing training and/or clinical direction as applicable to supporting government HCWs (e.g., hospital corpsmen, students, etc.). uf0b7 Attend and participate in various meetings as directed. uf0b7 Perform timely, accurate, and concise documentation of patient care. uf0b7 Operate and manipulate automated systems such as ADS, MHS Genesis participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications. uf0b7 Maintain documentation of all treatment provided in accordance with clinic directives and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions. uf0b7 Assess each patient and perform triage duties as assigned. uf0b7 Formulate and carry out a goal directed plan of care which based on determined nursing diagnosis and patient outcomes, and which is prioritized according to patient needs and available resources including time, personnel, equipment, and supplies. uf0b7 Evaluate effectiveness of self-care given by all health team members, and contribution of systems, environment, and instrumentation in progressing patient toward outcomes. uf0b7 Provide treatment and discharge instructions upon patient release. uf0b7 Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, psychological, and cognitive status. uf0b7 Provide a safe and clean environment for each patient. uf0b7 Identify patient/significant other learning needs and implement appropriate measures to meet identified needs. uf0b7 Assist in planning, provide clinical direction and provide instruction to LPNs/LVNs, nursing assistants and ancillary personnel. uf0b7 Follow up with patient referred by Primary Care Manager (PCM) or Internal, Behavior Health Consultant (IBHC) and focus on patient progress in PCM treatment adherence. uf0b7 As required, participate in PCM or IBHC case staffing and consultations that involve patients actively engaged in or may benefit in care facilitator services. uf0b7 Participate in screening processes developed by the IBHC and PCM to improve the recognition of and appropriate interventions for depression and PTSD. uf0b7 As required, provide, and teach curriculum content for classes that promote education and skill-building to enhance enrollee population psychological and physical health. uf0b7 As part of the MHP team, the HCWs shall raise awareness about behavioral health issues and care facilitator services in MHP practice area to promote easy referral of patients. uf0b7 Inform the Clinic Manager of administrative/clinical problems as they arise. uf0b7 Participate in all accreditation and Medical Home Port recognition processes (JC, IG, etc.). uf0b7 Engage other MHP team members to assist in improving documentation of the episode of care using MHP Clinic-approved templates as the initial template (i.e., Tri- Service Workflow/COMPASS). uf0b7 Coordinate schedule and non-availability (i.e., leave, training, etc.) with other BHIPMHP team members to ensure adequate access to care. uf0b7 Participate in MHP team huddles. Qualifications Minimum Qualifications: Education: Minimum ADN from an accredited college or university Experience: Possess a minimum of 1 year outpatient experience as an ambulatory care nurse within the last 2 years, with experience providing services in psychiatric nursing, behavioral health and/or wellness counseling. Licensure: Current, full, active, and unrestricted RN license from any state. Life Support Certifications: AHA or ARC BLS Certification Security: Must possess ability to pass a Government background check/security clearance. _The salary range for this position is $45.00hr. - $75/hr.
