Summary The Cardiology Procedure Coordinator (CPC) Registered Nurse (RN) facilitates pre and post procedure care to patients undergoing invasive diagnostic and therapeutic cardiac catheterization procedures, and other invasive and non-invasive cardiac procedures in the Cardiac/EP Lab. This RN serves as a liaison between cardiologists, patients, families, VISN wide Referral Case Managers, EP nurses and nursing homes. Responsibilities Major Duties include but are not limited to: Facilitates pre and post procedure care to patients undergoing invasive diagnostic and therapeutic cardiac catheterization procedures, and other invasive and non-invasive cardiac procedures in the Cardiac/EP Lab. Serves as a liaison between cardiologists, patients, families, VISN wide Referral Case Managers, EP nurses and nursing homes. Collaborates with the Team (patient, Cardiology Provider, Primary Care Provider, and PACT) Collaborates with the Larger Team (family/caregiver, internal and community-based services involved in providing care to the patient). Evaluation of outpatient referrals, collection of results, communication to vendors, patients, and providers is essential. Triages and applies a collaborative team approach in identifying, analyzing, and resolving patient care problems Facilitation of Homecare/Community referrals and recommendations to support ongoing veteran needs Works collaboratively with all levels of health care professional and support staff to facilitate positive patient care outcomes VA offers a comprehensive total rewards package: VA Nurse Total Rewards Pay: Competitive salary, regular salary increases, potential for performance awards Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory Work Schedule: Start time between 0700-0830 - Monday - Friday - Schedule will be discussed and agreed between applicant and hiring manager Compressed/Flexible: Not Authorized Telework: Not Authorized Virtual: This is not a virtual position.
