JOB REQUIREMENTS: Group Health Cooperative of South Central Wisconsin.BETTER TOGETHER 1.0 FTE / 40 Hours per week The SPARC Behavioral HealthClinical Nurse - SPARC (BHN) participates as an integral member of thesubspecialty SPARC clinic. The SPARC BHN performs clinical,administrative and technical tasks which contribute to the smoothworkflow and high-quality patient care experience primarily within theSPARC subspecialty clinic with some additional responsibilities to theoutpatient psychiatric providers and BHN team. The primary function ofthe SPARC BHN will be to administer and record results of the DIVA-5 orother semi-structured interview and screening tools and to communicatethose results via EMR and discussion to the SPARC team or referringoutpatient psychiatric provider. The SPARC BHN will also provide overallsupport to the SPARC team to ensure coordination, delivery, andcommunication of care provided within the clinic to referring providers.Responsibilities will also include, but are not limited to: supportingEpic in-basket workflow by reviewing team/provider in-baskets andMyChart messages; coordinating the review and follow-up of phonerequests for medical advice and prescription refills within SPARC;completing and documenting reviews of controlled substance prescriptionswithin the Prescription Drug Monitoring Program; returning phone calls,completion pre-visit preparation; monitoring patient flow; assistingwith triage and patient scheduling; and coordinating with otherdepartments, services and programs relevant to the SPARC workflow andthose referred for DIVA 5 by outpatient psychiatry team. Psychiatricproviders and the Behavioral Health Nurse Manager may delegateassignments to the SPARC BHN. SPARC BHN at GHC-SCW will have a degree innursing and have an active and current nursing license.
