JOB REQUIREMENTS: Provides case management through CLTS Support andService Coordination to children who are diagnosed with physical,emotional, or cognitive disabilities. The goal of the CLTS WaiverProgram is to support children with substantial needs, as well as theirparents/guardians, by delivering services to promote the goal of everychild living their best possible life now and into the future. Thisposition may be eligible for a hybrid schedule to telework up to 3 daysa week after successful completion of an initial employment period.Essential Duties Support and Service Coordination assists a child oryouth and his or her family to achieve an inclusive, independent, andself-empowered life. Support and Service Coordinators use theirknowledge of available programs, resources, and services to create anindividualized service plan that best supports the child and family inpursuit of their desired outcomes. This includes connecting families tosupports throughout the community and promotes family members'self-determination and involvement in all facets of community life. At aminimum, monthly collateral contacts and direct contact with familyevery three months must occur. Assisting to establish and maintain allaspects of program functional and financial eligibility. Providesparticipant and family education, support, and advocacy regarding theirdisability. Assessing and periodically reassessing the child's oryouth's health, safety, and functional capacity. Reports incidents thatcreate a significant risk or services harm to the physical, mentalhealth, safety, or well-being of a child to the WI Department of HealthServices. Assures the application of client rights, grievance anddispute resolution processes and procedures. Completes authorizationsfor purchase of service and obtains supervisory approval in line withservice plans. Monitors contracted services to assure high quality andthe appropriate extent of services provided. Interfaces with healthcareproviders/insurers/Medical Assistance/Social Security to coordinateservices. Assists families with transition to adulthood process andconnecting them to resources related to needs. Identifies the need forand assists in the planning and development of new service resources asnecessary. Complies with applicable county, state and federal guidelinesrelated to the provision of service. Completes and maintains accurateand timely documentation according to applicable requirements, codes,contracts and policies. Facilitates collaboration and maintains positiveworking relationships with and between participants, informal supportsystems, co-workers, health professionals, courts, legalrepresentatives, contracted providers and the public at large. Attendsand contributes to staff meetings and overall unit performance.Functions as part of treatment teams/multi-disciplinary teams to ensurecontinuity of care. NON-ESSENTIAL DUTIES: Performs related functions asassigned. MATERIALS AND EQUIPMENT USED: General office equipmentComputer PHYSICAL DEMANDS: Lifting 20 pounds maximum with frequentlifting and/or carrying of objects weighing up to 10 termittent standing, walking, and sitting; occasional driving. Fieldwork requiring face to face and collateral contacts located in home andcommunity-based settings. Using hand(s)/feet for repetitive singlegrasping, fine manipulation, pushing and pulling, and operatingcontrols. Occasional bending, twisting, squatting, climbing, reaching,and grappling. Distinguishing sounds at various frequencies and volumes.Distinguishing people or objects at varied distances under a variety oflight conditions. Knowledge of dynamics involved incommunity-based care for persons with long-term care needs. Knowledge ofsocial work assessment principles and techniques. Knowledge of clientrights, grievance and dispute resolution processes and procedures.Knowledge of state and federal laws, codes, procedures, and policiespertaining to Medicaid and Wisconsin Long-Term Care program delivery.Knowledge of community resources. Knowledge of needs and dynamics ofthose with all types of disabilities. Ability to establish prioritiesfor service coordination and to consider alternative treatment plans.Ability to work independently. Ability to use receive and providefeedback. Ability to be decisive and to make sound decisions under thepressure of urgent situations. Ability to plan and organize work to meetdeadlines and to make effective use of time. Ability to establish andmaintain positive relationships with consumers and their family members,staff, contracted providers, and the public. Ability to communicateeffectively both orally and in writing. Ability to work the requiredhours of the position, which is 37.5 hours weekly, with core hours being8:00am-4:30pm.
