JOB SUMMARY: We are seeking anexperienced, bilingual English/Spanish professional to join our team asa Service Coordinator for the Children's Long-Term Support (CLTS)Waiver Program. As a Service Coordinator, you will provide casemanagement and service coordination services to families and individualsneeding counseling and referral services for one or more of thefollowing: intellectual and/or developmental, physical, or mental healthdisabilities. Conduct assessments of client needsthrough interviews, collateral contacts, and other investigations.Develop a casework plan with clients and perform casework services.Maintain a caseload involving clients with personal, family, orsocio-economic problems and attend to their related protection andsafety needs. Counsel and provide intervention and case managementservices by coordinating various social, community, and other servicesreceived by the clients. Complete in-person home visits to conductassessments and maintain client contact. LOCATION: Waukesha County HumanServices building. Local travel is required for client home visits, withmileage reimbursement. Service Coordinators have the opportunity to workhybrid/WFH after six months.
