This position is a 100% onsite opportunity. This convenient location offers comprehensive primary care for the whole family with experts in family medicine, internal medicine, obstetrics and gynecology, and general pediatrics.A Registered Nurse, UCM Care Network, is responsible for providing professional nursing care for patients including assessment, treatment, care planning, and medical care evaluation by following established standards, protocols and practices. Assists physicians and staff in coordinating patient care. Functions as primary liaison between patients, staff and physicians. This position will be located in a community-based physician practice.Essential Job FunctionsObtains patient consent for care and ensures patient confidentialityObtains and documents health history, information on chief complaint, vital signs, and health risk factors and assesses disease conditionProvides patient care based on practice guidelines, standards of care, and federal/state laws and regulationsExecutes prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient responseDelegates as allowed under the Nurse Practice Act. Refers to/consults with physicians, other health providers, and community resources to prevent/resolve problems or concernsEducates patients and families about health status, health maintenance, and management of acute and chronic conditionsDocuments patient assessment and intervention data using established medical record forms/automated systems and documentation practicesParticipates in multidisciplinary teams to improve patient care processes and outcomesTracks quality assurance data and monitors for acute and chronic care managementMaintains/reviews patient records, charts and other pertinent information Posts tests and examination resultsResponds to/refers incoming patient treatment-related phone callsMaintains patient confidentialityPerforms other duties assignedRequired QualificationsMust be an actively licensed Registered Nurse in the state of IllinoisMinimum of one year professional nursing experienceCompletion of nursing training from an accredited school of nursingActive BLS certification from the American Heart Association (AHA)Preferred QualificationsABachelor's degree in Nursing Prior clinic experience Position DetailsJob Type/FTE: Part-Time 0.5 FTEShift: Days - 8 hour shifts or 12 hour shifts, 2 weekend shifts per monthWork Location: Onsite - Dearborn StationUnit/Department: Urgent CareCBA Code:Non-UnionWe’ve been at the forefront of medicine since 1899.
