We want your knowledge and expertise!Job Summary and QualificationsAs a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient-centered care. They are dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Specialized assessment skills help ensure that patients are placed inthe appropriatecare areas and discharged. We offer education and post-hospitalizationcareas necessary. Theirmain focusis advocating for patients, families, and caregivers while embodying the organization's vision, mission, and values. The goal is to give an unmatched patient experience andattainoptimalclinical outcomes. What will you do in this role:Obtains a nursing assessmenthistory, andensures proper inpatient care unit admission.Takesappropriate actionwhen needed, such as communicatingspecial circumstancesto physicians and/or care team or reporting indications of abuse or pletes andfacilitatesthe completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.Teachespatients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications andpossible sideeffects, and follow-up measures. Translates complex medical terminology to ensure complete pletes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.Ensurespatients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.Coordinates with House Supervisors/Nursing Administration to prioritize workload andidentifygreatestneed areas to ensure smooth patient transitions.Demonstratesa time-conscious awareness and ability to prioritize. Distinguishes between low and high-priority activities.What qualifications you will need:Advanced Cardiac LifeSptmust be obtained within60 monthsof employment start dateBasic Cardiac Life Support must be obtained within30 daysof employment start dateRegistered NurselicenseAssociate Degree, or Registered Nurse Diploma, orBachelorsDegreeParkridge Medical Center is a 270+ bed hospital. We are near downtown Chattanooga. We feature comprehensive diagnostic, medical, and surgical services. These services include cardiovascular surgery and cancer treatment. These services include intensive care, orthopedic and spine surgery, and rehabilitation. The facility includes a state of the art neurosurgery program. Patients also have access to OB-GYN services. Patients have access to a level III Neonatal Intensive Care Unit. They also have access to (NICU) and behavioral health treatment centers for children and adults.Bricks and mortar do not make a hospital.
