Description u2022 Administer u2022 Ask the patient a screening question about pain if they have any pain (Yes/No) additional assessment needs to be done by the RN. u2022 Any other functions that a PCT can perform (i.e. V/S, I+Os weights, ambulation, etc) u2022 Answer patient call signals, signal lights, bells, or intercom systems to determine patients' needs. u2022 Turn or reposition bedridden patients. u2022 Provide physical support to assist patients to perform daily living activities, such as getting out of bed, bathing, dressing, using the toilet, and standing, walking, or exercising. u2022 Review patients' dietary restrictions, food allergies, and preferences to ensure patient receives appropriate diet. u2022 Measure and record food and liquid intake or urinary and fecal output, reporting changes to medical or nursing staff. u2022 Record vital signs, such as temperature, blood pressure, pulse, or respiration rate, as directed by medical or nursing staff. u2022 Gather information from caregivers, nurses, or physicians about patient condition, treatment plans, or appropriate activities. u2022 Observe or examine patients to detect symptoms that may require medical attention, such as bruises, open wounds, cognition, abnormal vital signs, and any signs of distress or blood in urine. u2022 Document or otherwise report observations of patient behavior, complaints, or physical symptoms to nurses. u2022 Remind patients to take medications or nutritional supplements. u2022 Assist with rounding.
