What We Offer Clinical Service Navigator (Registered Nurse) + Schedule: 40 hours per week, Monday through Friday 8-5pm + Location: Forsyth Medical Center + Patient Population: COPD and Heart Failure Patients Serves as primary liaison between the patient and his/her medical team as well as patient advocate and support. Provide referral sources and support services to patients to work towards lowering patients' readmissions.
