FTE: 0.875000 Standard Hours: 35.00Shift: Shift 1 Shift Details: 7/70 schedule, 10 hour shifts This rolewill focus on hospital-based inpatient unit work. Job Summary: The RNCase Manager coordinates the provision of care and services of patientson the Inpatient Rehabilitation Unit through their stay and across thecontinuum. The RN Case Manager works collaboratively withinterdisciplinary teams and team members both internal and external tothe organization to improve patient care by assuming a leadership rolein driving desired clinical and financial outcomes through effectiveutilization and monitoring of healthcare resources. The RN Case Managerutilizes standards, guidelines, and protocols for care delivery andincorporates data to continuously improve care and outcomes. EXPERIENCEDESCRIPTION: A minimum of 3 years of clinical nursing is required. 5years of clinical nursing and case management experience is preferred.Experience in case management field including transitional planning,outcomes management and assessment preferred. EDUCATION DESCRIPTION:Professional knowledge of nursing theory and practice at a levelnormally acquired through completion of a minimum of two years educationat an accredited School of Nursing in order to be eligible for licensureas a Registered Nurse is required. Bachelor's degree is preferred.TRAINING DESCRIPTION: Accredited case manager certification (ACM-RN)through the American Case Management Association (ACMA) preferred.SPECIAL SKILLS DESCRIPTION: General knowledge of payor industry(Medicare, Medicaid, & Commercial insurance regulations), resourcemanagement, and evidenced based clinical practice is essential.Theoretical knowledge of the nursing and case management processes.Analytical skills necessary to assess patient's discharge needs anddevelop solutions to problems concerning patients' mental status orphysical well-being to ensure optimal patient outcomes. Advancedcommunication and interpersonal skills with all levels of internal andexternal customers including but not limited to medical staff, patientsand families, clinical personnel, support and technical staff, outsideagencies, and members of the community. Other skills needed includeexcellent written/verbal communication skills, critical thinking,creative problem solving, proficient organization and planning. Basiccomputer skills. Must be self-directed with ability to tolerate frequentinterruptions and a demanding workload. LICENSURE DESCRIPTION: Requirescurrent state of Wisconsin Registered Nurse License or a Multi-stateNursing License from a participating state in the NLC (Nurse LicensureCompact).
