The Community Health Navigator provides culturally responsive, harm-reduction-oriented outreach, navigation, and care coordination services to individuals who use drugs, with a focus on Black, Indigenous, and People of Color (BIPOC) communities disproportionately impacted by substance use, overdose, homelessness, and structural inequities.
