Provide clinical recommendations to physicians andfacility staff regarding medication therapy changes and optimizations.Maintain up-to-date patient/resident profiles, including medicalhistory, lab results, and current medications (prescription, OTC, andhome remedies). Ensure laboratory tests required for monitoringhigh-risk medications are appropriately ordered, reviewed, andcommunicated to the healthcare team. Collaborate with physicians,nurses, and other healthcare professionals to provide clinical andtechnical support. Document all communications and clinicalinterventions in the patient's medical record, including interactionswith families and caregivers. Implement and enforce policies andprocedures for the safe procurement, storage, administration, anddisposal of medications and biologicals.
