Travel Registered Nurse u2013 Hospice

Weu2019re looking for a Hospice RN u2013 Regional Support to join our team. In this dynamic role, youu2019ll provide direct care to patients and support to families, while also serving as a floating clinicianu2014temporarily assigned to different hospice branches based on census needs or staffing gaps. Territory to include: Crossville, Knoxville, Chattanooga, Cleveland, Athens, Morristown, Greeneville, Johnson City and Kingsport As a Hospice RN u2013 Regional Support, You Will: Deliver high-quality, compassionate nursing care for hospice patients Manage and implement individualized plans of care Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.) Assess and address patientsu2019 physical, emotional, spiritual, and environmental needs Accurately document all care provided and ensure compliance with hospice protocols Educate families and caregivers on care instructions, medications, and safety Participate in the on-call rotation and provide after-hours care as needed Collaborate with physicians, referral sources, and the care team Assist with referrals, regulatory documentation, and payer compliance Travel to various Gentiva hospice locations as assigned to fill urgent needs Schedule & Travel: This role typically follows a Mondayu2013Friday schedule, with occasional on-call responsibilities.
