Job Analysis Job Title PT Home Health Job Summary Home Health Physical Therapist needed to provide physical therapy services to patients in their homes within a designated service area in Washington D.C. Requires HomeCare Homebase/OASIS experience. Expected to manage approximately 6 visits per day, utilizing a personal vehicle for travel and accurately tracking mileage. Key Responsibilities Provide physical therapy services to patients in their homes. Assess patient conditions and develop appropriate treatment plans. Implement and monitor patient progress with treatment plans. Educate patients and their families on exercises and treatment protocols. Maintain accurate and timely documentation using HomeCare Homebase/OASIS. Coordinate with other healthcare professionals to ensure comprehensive patient care. Adhere to all facility and regulatory guidelines. May be required to cover other zip codes in the coverage area based on staffing needs. Required Qualifications Minimum of 2 years of experience as a Physical Therapist. HomeCare Homebase/OASIS experience required. BLS certification. DC license in hand at the time of submission. Reliable vehicle in good standing with all licensure and insurance requirements. Preferred Qualifications Previous travel experience. Licensure (Required at Time of Submission) DC Locations: Must have a DC license in hand. Some units will accept a pending DC license (see job description to confirm). A later start date may be accepted if the traveler has applied for DC licensure. MD Locations: Must have either a compact license or an MD license.
