DescriptionThis is an open and continuous posting for full-time and part-time positions on multiple shifts serving the Group Homes throughout the Metro Region of DDS.TheDepartment of Developmental Services (DDS)seeks a caring and compassionate applicant for the role ofLicensed Practical Nurse II(LPN II) who is committed to building meaningful relationships in order to provide outstanding care and service. The LPN II participates in the delivery of health care services and other support services to individuals with developmental disabilities in homes in the community. This candidate works to promote good health, prevention of illness, and support individuals to maximize their independence and become active members of their communities. LPN II works closely with direct support staff and are considered a part of coverage. The candidate also monitors Medication Administration and completes necessary documentation as requested.Please Note: Based on assignment, candidates may be eligible for various pay differentials.For more information on the Department of Developmental Services (DDS) go to:To visit the DDS Career Page, please visit:Licensed Practical Nurse II POSITIONS AVAILABLE IN THE FOLLOWINGCITIES & TOWNS:Ashland - Canton - Charlestown - Dedham - Dorchester - Foxboro - Framingham - Hyde Park - Medfield - Needham - Norfolk - Roslindale - Roxbury/Mission Hill - South Boston - Somerville - Walpole - Waltham - Wellesley - Westboro - WrenthamDuties and Responsibilities (these duties are a general summary and not all inclusive):Provide exceptional nursing care and oversight by implementing the individuals' health care plan including charting vital signs, application of medical/surgical dressings, and administering medications as prescribed and directed by the primary/specialty physicianAssist with daily maintenance, personal care and other services of support as outlined in the Individual Service Plan (ISP); Maintain individual records and filesInitiate referrals for needed medical services including scheduling, hospital safeguarding, advocacy and other follow-up care; Engage in referral for supportive and health-related protectionsCommunicate health issues to staff and update the individuals' families/guardians in matters including hospitalizations, rehabilitation support such as physical/occupational therapyInform staff of any medical concerns related to activity level that may hinder and individual's progressOrder medications and maintain medical supplies, materials and equipmentInitiate emergency medical procedures by applying basic first aid techniques and/or CPR and tracheotomy care, and catheterizationsProvide training for direct support staff on use of equipment and treatmentsTransport individuals to work, medical appointments, recreational activities and other community engagements as neededPosition Requirements:Must possess a current, active license to practice as an LPN in the Commonwealth of Massachusetts.Agency Requirements:Must possess a current and valid Class D Motor Vehicle Operator's LicenseIncumbents must obtain and maintain Health Care Provider Basic Life Support (BLS) certificationPreferred Qualifications:Demonstrated ability to develop and maintain a respectful, caring, tolerant, ethical, and empowering relationship with individuals with intellectual and developmental disabilitiesCapacity to assist/lift individuals with physical disabilities and/or limited mobilityKnowledge of standard nursing policies, procedures, methods and techniquesCapacity to maintain accurate records and exercise discretion in handling confidential informationFamiliarity with safety practices and medical terminology/abbreviationsAptitude to transcribe, read and comprehend the English languagePlease Note:Depending on assignment, some LPN positions may be assigned to locations that require 24/7 nursing staff. The Department of Developmental Services requires employees in these positions to be able to perform mandatory overtime when circumstances dictate the need.This position is designated as an "EMERGENCY" position in the Department of Developmental Services Emergency Plan.
