JOB DESCRIPTION Job SummaryProvides support for mobile unit activities including support. Responsible for routine and complex transportation services related to driving and transport coordination, in addition to activities related to connecting members to community services and resources. Contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties u2022 Responsible for driving mobile unit to scheduled community events/locations/destinations to support member care. u2022 Inspects mobile unit for defects before and after trips and reports any discrepancies indicating mobile unit condition to leadership. u2022 Maneuvers mobile unit as needed, includes backing unit in and out, navigating around tight corners, manufacturing areas and high pedestrian traffic. u2022 Provides minor vehicle maintenance, (i.e. maintains fluid levels and keeps unit clean inside and out). u2022 Performs driving duties in conformance to appropriate safety and security standards. u2022 Assesses eligibility for mobile medical services, and provides explanation and instruction on how to access services. u2022 Registers member in mobile services electronic medical record (EMR) system (Epic), and schedules services as needed. u2022 Runs required reports and transmits discharge summaries to identified providers. u2022 Assists members in completing enrollment paperwork for mobile services, and coordinating with enrollment team, program clinicians and providers. u2022 Evaluates and resolves issues related to member ability to access services on the mobile medical unit. u2022 Educates other agencies about related mobile services and access criteria. u2022 Establishes and maintains relationships with community and social resources. u2022 Provides technical assistance, guidance and direction staff as needed. u2022 Up to 75% local travel may be required (based upon state/contractual requirements). Required Qualificationsu2022 At least 1 year of related experience related to transport, customer service, and or health promotion/health education, or equivalent combination of relevant education and experience that demonstrates ability to do the job. u2022 Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements. u2022 Ability to drive a 39u2019 Class A Winnebago. u2022 Thorough knowledge of traffic laws and defensive driving. u2022 Ability to operate required tools and equipment. u2022 Ability to safely drive various business vehicles. u2022 Ability to establish and maintain effective relationships with employees, leaders, members and the general public. u2022 Ability to communicate effectively, work independently and make independent judgments which have moderate impacts on the organization. u2022 Ability to perform heavy manual tasks under varying weather conditions. u2022 Strong verbal and written communication skills. u2022 Microsoft Office suite/applicable software program(s) proficiency, and ability to learn new programs. u2022 Ability to lift 40 lbs.
