DescriptionPOSITION PURPOSEThe Resource Navigator connects clients with essential services and support. Whether assisting with obtaining areplacement ID, applying for benefits, or securing housing referrals, this role ensures individuals are linked tothe resources they need to achieve selfsufficiency.ESSENTIAL FUNCTIONS AND RESPONSIBILITIESTo be successful in this role, an individual must be able to perform the functions listed below satisfactorily. The Townshipwill make reasonable accommodations to enable individuals with disabilities to perform these pletes any job assignment that may be deemed necessary or appropriate to fulfill the mission of theTownship.Reacts to change productively and handles other tasks as municates with clients and the public in a courteous and respectful manner.Conduct individualized needs assessments, case management and advocacy with clients.Provide resource referrals.Advocate with other service providers or systems as needed.Assist with accessing services as needed (for example, social security benefits and EBT)Utilize evidencebased interventions as appropriate.Follow up with clients to ensure that individuals are successfully connected with services.Maintain accurate and timely records.Maintain connected relationships with partner piles operational statistics and gathers such other data as the Chief of Staff may require.Stays uptodate on current programs, policies, and resources available to assist individuals.Act as a liaison between individuals and community resources, such as healthcare providers,government agencies, and nonprofits.Provide inperson, telephone, and email support to individuals as needed.Maintain accurate records of all interactions and case management activities.Conduct thorough assessments of individuals needs and resources to determine the best course ofaction.SUPERVISORY RESPONSIBILITIESThis position has no direct reports or supervisory responsibilities.
