Job Description u00b7 Assigned to specific zip codes and partnered with one Nurse Practitioner, covering a broad geographic area. (Northside OR Westside) u2003u00b7 Responsible for reconciliation tasks and conducting patient screenings. u2003u00b7 Assist the Nurse Practitioner with any required orders. u2003u00b7 Will eventually be responsible for drawing blood and transporting samples to the lab. u2003u00b7 Service area includes the 32210 region and Green Cove. u2003u00b7 Will interact with a diverse patient population, including individuals and families, some of whom may have dementia or other health concerns. u2003u00b7 Expected to schedule and complete five patient visits per day. u2003u00b7 Schedule 5 visits a day We are a company committed to creating diverse and inclusive environments where people can bring their full, authentic selves to work every day.
