About the Role Weu2019re looking for a compassionate and detailu2011oriented Outpatient Care Coordinator to support our Case Management team. In this nonu2011clinical role, youu2019ll help coordinate services and administrative tasks that ensure members receive timely, organized, and highu2011quality care. What Youu2019ll Be Doing + Support Outpatient Case Managers by coordinating nonu2011clinical services for members who meet case management criteria + Assist with scheduling medical appointments, specialist visits, home health services, durable medical equipment (DME), and transportation + Perform eligibility and benefit verification and request medical records as needed + Conduct outreach calls to members to introduce and invite them into the Case Management program + Follow up with members to ensure services are received as scheduled + Maintain accurate and reliable information in reports, data systems, and case records + Review medical management reports and identify or report discrepancies + Keep patients, families, and internal teams informed throughout the care coordination process Communication & Teamwork + Serve as a liaison between members, physicians, healthcare providers, and administrative teams + Communicate clearly and professionally with management, coworkers, and providers + Support a collaborative team environment by assisting others as needed + Demonstrate flexibility and a teamu2011player mindset to meet departmental needs Patient Experience & Professional Standards + Treat members and their families with respect, dignity, and empathy + Participate in service recovery by following up with upset members and addressing concerns + Demonstrate honesty, integrity, and accountability in daily work + Protect patient and employee privacy by maintaining strict confidentiality of all health information + Recognize and promptly report errors to the appropriate manager What Weu2019re Looking For + Strong attention to detail and ability to manage multiple tasks + Comfort with data entry, documentation, and reports + Ability to follow through on requests and meet turnaround time expectations + Strong phone communication skills and a patientu2011centered approach + Ability to type 35+ WPM and navigate multiple web browsers/systems simultaneously Job Type & Location This is a Contract position based out of Covina, CA. Pay and Benefits The pay range for this position is $23.00 - $23.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
