FTE: 1.000000 Shift: Shift 1 Job Summary: This position focuses onhealth care for individuals, families, groups, communities andpopulations which occurs in primary and specialty care outpatientclinics. Addresses in partnership and collaboration with other healthcare professionals the patient's wellness, acute illness, chronicdisease, disability, and end -of-life needs. Responsible for patientadvocacy, coordination of nursing and other health services,implementation of nursing services and continuity of care. Providespatient education when needed. JOB REQUIREMENTS/PREFERRENCES: Graduateof an accredited school of nursing required, and current unrestrictedWisconsin RN license required. None required; at least one yearmedical/surgical nursing preferred. CPR certification required.Excellent communication and organizational skills required.
