This position is in the HBPC section of the Geriatric & Extended Care, Dayton VA Medical Center, Dayton, Ohio. The Physical Therapist (PT) who works at this level provides assessment and treatment intervention on an independent basis across the continuum of care and services provided by the Care Line. Responsibilities VA Careers - Physical Therapy: Duties includes but are not limited to: Responsible for the examination and treatment of patients referred to the HBPC. Establishes and maintains contact with other key staff involved in patient care via written, telephone, and personal communication on a regular basis. Conducts comprehensive patient examinations to identify rehabilitation problems and establish treatment goals. Communicates effectively with other internal and external health care providers. Provide education to patients, family members, caregivers, employees, students, and coworkers. Scheduled appointments for assigned patients. Awareness of safety hazards and takes appropriate corrective action. Adheres to infection control programs. Provides training to clinical staff on new techniques and equipment that will assist in the evaluation of the home bound patient. Schedules and completes comprehensive evaluations on patients referred through the program. Work independently in the home solving complex problems relating to Physical Therapy, adapting and modifying the treatment plans, activities, and procedures to meet the needs of the patient with varying degrees of disabilities.
