Become a part of our caring community and help us put health first The Community Health Worker/Care Management Support Professional 1 contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Professional 1 work assignments are often straightforward and of moderate complexity. Humanau2019s Community Health Workers (CHW) serve as members of our Louisiana Medicaid enrollee centric Comprehensive Care Support teams. The CHW applies both a hands-on approach, as well as telephonic outreach, to enrollee engagement to support case management functions, address social determinants of health (SDOH) needs, improve self-management of chronic conditions, navigate the healthcare system, and promote prevention and health education that is tailored to the needs of the communities we serve. The CHW leverages their knowledge of the community and shared life experience to inform their interactions with enrollees and Humanau2019s community partners. This role reports to the Manager Care Management and be part of the Medicaid Team. In this role you will: u2022Collaborate with Humanau2019s Comprehensive Care Support team, including case managers, peer support specialists, housing specialists, and SDOH coordinators. u2022Conduct in-person and telephonic assessments to identify enrollee needs, preferences, and socioeconomic barriers, including home environment evaluations. u2022Identify and address barriers to healthy living and healthcare access; assist with scheduling physical and behavioral health appointments. u2022Advocate for enrollees with providers, community resources, and schools; accompany enrollees to provider visits as needed. u2022Monitor care plan compliance and use motivational interviewing to support medication and treatment adherence. u2022Provide consistent, non-judgmental social support to enhance enrolleesu2019 morale and self-worth within the multidisciplinary team. u2022Deliver culturally appropriate health education and coaching to promote self-management. u2022Participate in Humana community events to engage enrollees and educate on case management services. u2022Conduct outreach and research to locate hard-to-reach enrollees and increase participation in assessments and clinical programs. u2022Build relationships with providers and community resources; conduct community assessments to identify gaps and support referrals. Use your skills to make an impact Required Qualifications u2022Minimum two (2) years prior experience working with community resources, community health agencies/social service agencies (Area Agency on Aging, DME providers, Meals on Wheels etc.) u2022Proficient in using electronic information systems and Microsoft Office tools (Word, Outlook, Excel) to document, communicate, and collaborate effectively with internal partners and members. Demonstrates strong keyboard and web navigation skills, provides clear narrative documentation, and builds rapport through exceptional communication and interpersonal abilities. u2022Decision making skills regarding own work approach/priorities, and work assignments, standards and resources with the ability to multi-task and work in a very fast-paced environment u2022Strong understanding and respect of all cultures and demographic diversity. u2022Willing to travel up to 75% of the time to see members Preferred Qualifications u2022Bacheloru2019s Degree in Social Work or related field u2022Community Health Worker training and/or certificate or willingness to complete within one (1) year u2022Familiarity with state Medicaid program guidelines u2022Experience engaging with Medicaid enrollees, including those with physical health and behavioral health needs and varied health literacy u2022Fluent in Spanish, French Creole, or Vietnamese Work-At-Home Requirements To ensure Home or Hybrid Home/Office employeesu2019 ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: u2022At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is required. u2022Satellite, cellular and microwave connection can be used only if approved by leadership. u2022Employees who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense. u2022Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job. u2022Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information. Workstyle: Combination remote work at home and onsite/home member visits Hours: Must be able to work a 40 hour work week, Monday through Friday 8:00 AM to 5:00 PM, over-time may be requested to meet business needs and requires approval. Screening: This role is considered patient facing and is part of Humanau2019s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. Travel, Driver's License, Transportation, Insurance: Must be willing to travel at least 75% of the time within a 30-40 mile radius of your assigned Region, to conduct field visits with members in your assigned area. Travel requirements may vary based on member tier level. This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. u2022Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher. Mileage reimbursement is provided for work-related travel. Eligible mileage includes: u2022Travel from your home to your first work location of the day. u2022Travel between client or assignment locations during the workday. u2022Travel from your final work location back to your home. Additional Information Section 1121 of the Louisiana Code of Governmental Ethics states that current or former agency heads or elected officials, board or commission members or public employees of the Louisiana Health Department (LDH) who work directly with LDHu2019s Medicaid Division cannot be considered for this opportunity. Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
