Job DescriptionGeneral Purpose of Job:u00a0u00a0Provides direct physical therapy services under the direction and supervision of a licensed Physical Therapist according to the physician approved written plan of care.u00a0u00a0u00a0u00a0u00a0u00a0u00a0u00a0Essential Duties:This job description is intended to cover the minimum essential duties assigned on a regular basis.u00a0 Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.- Provides direct physical therapy treatment to patients according to instruction of Physical Therapist, and in accordance with the written plan of care- Observes patient and reports to Physical Therapist and department personnel the patient's reaction to treatment and any changes in patient's condition- Instructs patients, family and/or caregiver in proper techniques to assist patient in achieving maximum level of independence- Maintains necessary records, progress notes, and conference notes, which are incorporated into the patient's clinical record within established time frame- Maintains professional standards, compliance with State, Federal (CMS conditions of participation) and Joint Commission regulations, and infection control protocol by participating in in-services, staff meetings, process improvement teams, continuing education seminars when available, and adhering to the SHC expectations -- standards of care guidelinesu00a0u00a0u00a0 Job Requirements ---------------------------------- ----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------- General Requirements u2022 PTA license to practice in State of Michigan u2022 Valid driver's license and automobile insurance u2022 Active American Heart Association BLS Certification upon hire Work Experience u2022 Two years acute care or home care experience Education u2022 Graduate of a Physical Therapy Assistant program Specialized Knowledge and Skills Required: u2022 Demonstrated ability to work with patient, caregivers, and others in a variety of home care situation u2022 Demonstrated interpersonal skills to effectively and appropriately communicate with patients, families, physicians, and interdisciplinary team u2022 Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job u2022 Demonstrated ability to assemble and document data in proper format u2022 Ability to enter/exit patient homes that may/may not be handicap accessible Preferred: u2022 Home Care experience ---------------------------------- -----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------u00a0University of Michigan Health-Sparrow is an Equal Opportunity/Affirmative Action employer.
