Come make a difference at Ochsner Health and discover your future today Position Highlights: u2022 Provide an extra layer of support to patients with serious illness u2022 Consultations: inpatient, embedded supportive oncology clinic, free standing outpatient clinic, and post-acute facilities (LTACH/Rehab/SNF). u2022 Provide medical care related to symptom management. u2022 Participates in interdisciplinary team to develop a written plan of care. u2022 Develop and facilitates goals of care conversations for patients in the inpatient and outpatient setting. u2022 Actively participates in process improvement related to system and department goals. u2022 Participate in specialty team meetings. u2022 Serve as collaborating physician to advanced practice providers u2022 Staff inpatient hospital and outpatient clinic with advance practice providers and other members of the interdisciplinary team. u2022 EPIC electronic health record system at all Ochsner facilities.
