The UnityPoint Clinic team is known for excellence and quality patient care putting patients at the center of their care to provide the best level of health and well-being. + Established team of 2 physicians and 4 Advanced Practitioners + Diabetes Self-Management program staffed with Certified Diabetes Educators (RNu2019s and Dietitians) + Healthy Living Program, a medically managed weight loss program supported by Dietitians and a Health Coach + Experienced staff, both clinical and non-clinical, who know the patients and the community + Full-time position, including dedicated administrative time + 24/7 hospitalist program is responsible for adult inpatients + Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination Responsibilities + Outpatient clinic only; Adult and adolescent patients 15 + + Clinic hours Monday u2013 Friday 7AM-5PM + No nights or weekends + Shared call amongst the providers.
