Saint Joseph PACE has an excellent full-time employment opportunity for a Board Certified Family Medicine or Internal Medicine physician with Fellowship training or Certificate of Added Qualifications in Geriatric Medicine to become Medical Director of a Program of All-Inclusive Care for the Elderly (PACE) located in Mishawaka, Indiana.Job Description:Under the supervision of the Executive Director.Provides general medical direction and supervision of the medical aspects of participant careResponsible for the delivery of participant care, clinical outcomes, and oversight of the Quality Assurance Performance Improvement (QAPI) program. Responsible for the development and maintenance of medical policies and procedures/standards and protocolsProvides direction and supervision to related Primary Care Providers (PCPs) and APPs.Provides leadership and guidance to the program colleagues and providers in the local communityNeed to have a driver's license, proof of insurance, and means of transportationProficient in Microsoft Suite (Word, Excel, PowerPoint, etc.), in addition to basic knowledge of email, calendaring systems, and other software as required to perform the essential functions of the jobExcellent organizational, customer service, flexibility, and problem-solving skillsWillingness to travel within the region periodically as neededConduct history and physical on new participants and event-based and periodic reassessments. Develop medical treatment plan and provide medically necessary services in the range of locations in which participants may terview and evaluate applicants to PACE Organization and their caregivers and participate in the Intake and Assessment decision.Discuss each participant's medical conditions and prognosis with him/her and their caregiver to assist them in expressing their wishes and planning their future care.Execute assignments in a culture that is shared and collaborative across all divisions within TH PACE.Collaborate in the nursing treatment program of participants.Establish and/or maintain as a member of the leadership team a plan to achieve operating goals.Participate as an active member of PACE Organization interdisciplinary team. Educates and advises the team regarding the implications of diagnoses, cognitive and physical function and treatment options.Participate in team and family conferences and decision-making processes.Educate participants and families regarding health maintenance and their chronic medical conditions, in cooperation with nursing staff.Supervise the health care and treatment program as developed by the interdisciplinary team.Perform physical examinations, order and evaluate appropriate laboratory and diagnostic tests, and record results in a systematic manner.Effective skills in physical assessment and chronic disease management for frail older adults.Desire to work in a creative alternate health care setting.Able to manage changing priorities according to participant needs.Participates in provider on-call rotation as required.Minimum of 1 year experience working with the frail or elderly population is required.Not accepting visa candidatesProvider must be interested in Perm (not accepting Locums)See attached "Practice Description" for full job description ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.Board Certified in Internal Medicine or Family Medicine - REQUIREDBoard Certified in Geriatrics (Fellowship-trained) or Certificate of Added Qualifications in Geriatric Medicine (to be completed prior to start) - HIGHLY PREFERREDIN license (or willing to obtain) - REQUIREDClean Malpractice - HIGHLY PREFERREDCDS Certification (by time of start) - REQUIREDDEA Verbal Confirmation (by time of start) - REQUIREDBLS (by time of start) - REQUIREDMinimum of 1 year experience working with the frail or elderly population (add to "Experience" field) - REQUIREDPrevious supervisory experience (add to "Experience" field) - REQUIREDDoes the provider require visa support - J1 or H1B?
