ESSENTIAL FUNCTIONS: uf0b7 Provide outreach services to children and families uf0b7 Consult with team members in the development of individualized plans and multidisciplinary services for assigned clients uf0b7 Maintain client records completely and accurately to include IFSPu2019s with full assessment of client functioning and goals, documentation of client progress in the program, records of required contact with collaterals and family members, documentation of arena assessments and documentation of DPH forms and regulations regarding intake and discharge of clients uf0b7 Complete billing documentation according to schedule established by the company uf0b7 Adhere to professional standards and guidelines according to their respective disciplines uf0b7 Keep supervisor informed of progress of all clients uf0b7 Meet direct service requirements based on part time or full time status, as designated uf0b7 Adhere to clinical recommendations from clinical administration uf0b7 Maintain confidentiality in all clinical matters uf0b7 Remain compliant with billing and documentation in accordance with direct service requirement uf0b7 Conduct visits with clients and relevant collaterals in natural environments according to the needs of the child and family uf0b7 Develop group curricula for assigned groups as applicable. uf0b7 Participate in team meetings, supervision and in-service training as required uf0b7 Participate in program development and team goals uf0b7 Obtain necessary certifications as required (i.e. DEEC, EI certifications, etc.) and maintain licensure as applicable. uf0b7 Maintain contact with site and clients by checking voicemail and emails daily uf0b7 Other duties as required _Qualifications:_ + Salary range-$75,000-$80,000 + Graduate of an accredited RN program + Current MA state RN License + Current driver's license + Current CPR/First Aid certification + Excellent assessment skills with the ability to exercise critical thinking in the clinical environment + Effective communication skills u2013 ability to communicate with team members, medical professionals, those we serve, and their families + A reliable, responsible attitude and a compassionate approach + A commitment to quality in everything you do Join Our Team If you are passionate about making a difference in the lives of individuals and families, we encourage you to join Mentor South Bayu2019s mission of helping everyone live a full and happy life.
