Incorporates differences into the provision of care. Integrates knowledge, skills, and experiences to meet the needs of patients and families throughout the continuum to include patient and family education. + Responsible for developing, overseeing and implementing comprehensive case management and treatment plans for patients with cardiac and pulmonary diseases. + Utilizes the AIDET (Standard Patient Satisfaction Criteria) to communicate with others, with a focus on patients and their families. + Provides monitored exercise therapy to clients in the Lifestyle Management Program (i.e. with the diagnosis of heart disease, COPD, diabetes, PAD, obesity, etc.). + Maintains current clinical data, insurance information and program records on each client. + Adjusts treatment plan in accordance with established protocols, co-morbidities, client goals/needs and medical history.
